Illinois Tax Registration

Helpful links to register for Illinois withholding and unemployment accounts

Illinois Department of Revenue (Income Withholding)

  1. Illinois income withholding and unemployment accounts are registered by the agency using one combined application. You will only need to fill out this application once and will receive different IDs from each agency.
  2. Go to https://mytax.illinois.gov/_/
  3. Click on Register a New Businesss (Form REG-1)
  4. Complete the registration. If you have any questions, please reach out to the agency directly at 217-782-3336
  5. Once your registration process is completed, Fuse will need:
    1. Your account ID
      1. The Illinois Department of Revenue will use your company's Federal Identification Number (FEIN) followed by a Reference/Sequence number. 
      2. Reference/Sequence numbers are most commonly 000, but may sometimes be 001.
    1. Your filing frequency
    2. Power of Attorney
  1. Please forward the above items to support@fuseworkforce.com, and we will send you a prefilled Power of Attorney to complete.

 Illinois Department of Employment (Unemployment)

  1. Illinois income withholding and unemployment accounts are registered by the agency using one combined application. You will only need to fill out this application once. You will only need to fill out this application once and will receive different IDs from each agency.
  2. Go to https://mytax.illinois.gov/_/
  3. Click on Register a New Businesss (Form REG-1)
  4. Complete the registration. If you have any questions, please reach out to the agency directly at 800-247-4984
  5. Once your registration process is completed, Fuse will need:
    1. Your account ID
    2. Your unemployment rate
    3. Power of Attorney
  6. Please forward the above items to support@fuseworkforce.com, and we will send you a prefilled Power of Attorney to complete.
